After Hospital Care

At First In Caring, we understand that the period after a hospital stay can be a critical time for both NDIS participants and Aged Care clients. Our Nursing Care service is dedicated to providing comprehensive and personalised After Hospital Care to support individuals during their transition from the hospital back to their homes and communities.

When a client is discharged from the hospital, our nursing staff does an in-depth assessment to determine their medical condition, post-hospital care needs, and any particular instructions supplied by the hospital staff and their healthcare providers. This assessment assists us in developing a customised After Hospital Care plan that addresses the client’s specific needs and encourages their recovery.

We also focus on educating participants, clients and their families about post-hospital care instructions, medications, and any lifestyle modifications required for optimal recovery. Our nursing professionals take the time to explain the importance of adherence to prescribed treatments and the prevention of complications.
